Moving a lot of domains

I want to move all the domains I currently have with GoDaddy to 123-Reg.
A lot are .co.uk so does anyone know what the best procedure is for bulk transfers. Do I need a separate transfer sheet for each individual domain?

A also own some none 123-reg type extensions (.us for example). Do you think 123-reg would host them?
