Dynamic form generator/script?

Anyone know of any scripts or dynamic form generators which can be used for a contact form?

By dynamic, I mean the form field options change depending on what a user selects (e.g. on nearly all holiday sites, after selecting a country in a search form the next field for resorts automatically changes based on the country).

I wrote one of these for a member a while ago for carmakes I think, but I can't find the post, but http://www.downloadjavascripts.com/Combo_Boxes.aspx has plenty of options for chained selects.

Most holiday sites give you this on the white label, so you may not need to re-invent the wheel.
