Login to nominet

web domain manager

manage domains

Tick the name you want to transfer then near the top right under "Domain List for the "whatever" tag" it a button that says "Transfer"

Enter the new registrants details

After it's been put in their name release it to the new tag and done.

There are also EPP & Automaton commands to Create Account and Transfer if you want to build the functionality into your current systems
