How to transfer a domain to buyer?

Firstly, i am a noob & just started to register the first domain couple days ago on 123-reg.com

Assume that someone want to buy the domain from me, what can i do to transfer that one to them.

You can scan the completed forms and send them to the buyer. Then he can fill his parts and rescan them and send to the Nominet by email.
