third save the pdf to disk using the little save to disk item in acrobat reader (and not the file->save link of your browser)
at all times ignoring the adobes unnecessary warnings.
fourth reuse as necessary

Worked for me - it's basically a page with a hyperlink linking to http://devel.nominet.webdesign.co.uk/forms/domain_transfer_form.pdf

I think what's not working is the 'Get saveable PDF' button at the bottom of the page

Yeah... I just tried it and it gave an HTTP error: 'Bad Gateway' when I tried to submit... Lost all the infos!!

Please update if you can as it's a useful tool!!!
