Online petition

I'm setting up a local site for where I live, probably with a forum and looking to get a local playground revamped. I've been in contact with my local mp who said he agreed the playgorund needed updating but getting anything solid agreed is sloooooow to say the least.
Therefore I'm thinking of putting an online petition on the website not that anyone is likely to take notice. Is there any ready made ones out there or should I just do a form and db?
Also what info should I collect?

You could do it yourself or use a separate website like petition (my website) which is pretty well indexed and may help get extra coverage.

You can also include a link when you add the petition for your related forum

I have co-used http://www.petitiononline.com/ before now to collect and verify 10,000+ sigs.

Was able to use it and export for delivery to a local authority.
